PLEASE DO NOT mail or fax your form to our office after April 8 as it will NOT be processed.
Onsite Registration will begin Tuesday, April 30 5-8 pm
Remember, you must be a current member to receive member registration prices. If you have not yet paid your membership dues for 2013, you must do so prior to registering for the conference.

All Exhibit Booths must submit a complete Booth Representative Form to the Executive Office to pre-register your representatives. Please submit no later than April 5, 2013.
Changes, Additions and Deletions made to your booth representatives after you have submitted the Booth Representative Form (above) can be made by completed the Booth Rep. Add/Delete Form and submitting prior to April 5, 2013 or on-site after April 5.
